This was the case with some AT& T customers a while back. They went about the business of paying their cellular phone bills every month, when some mysterious charges began to appear on their phone bill.
It turns out that they were being charged for ringtones and other content that showed up on their wireless. To make it worse, they set the forms up in a way that authorizes them to make recurring charges on your wireless account. Since they show evidence to the Wireless carriers that you authorized the charges when you filled out the form, they get permission to make the charges.
